Other Perio Tasks

The following sections cover other tasks performed in the Perio Chart module. They include viewing perio statistics, managing patient consents, and accessing the Attachments/Consents module.

If configured by your institution, a patient’s perio statistics can be automatically calculated using a Crystal Report.

Important: Once an exam is completed and approved by a faculty member or instructor, it cannot be edited.

  1. Select a patient and open the Perio Chart module.
  2. Click the View Statistics () icon to display the Perio Statistics Crystal Report.

    3. Note: The Perio Statistics report uses the PerStats.rpt file.

  3. Review as necessary.

Consent forms are used in axiUm to record a patient’s acceptance of a variety of things including treatment(s), things re quired during the course of treatment, requests to release information, etc. After they are completed, consent forms are attached to the patient record.

If a consent form is missing, the Consent Required () indicator displays on the status bar.

  1. Select a patient and open the Perio Chart module.
  2. Click the Perio Treatment History () icon to display the Perio Tx History window.

  3. Click the Add Patient Consent () icon and select View Missing Required Consents… to display the Consent List window.

  4. If necessary, filter the listed results.
  5. (Optional) Select a listed consent and click the Preview Report () icon.

If a consent form is missing for a patient, you can choose to add one.

  1. Select a patient and open the Perio Chart module.
  2. Click the Perio Treatment History () icon to display the Perio Tx History window.

  3. Click the Add Patient Consent () icon and select Add Patient Consent… to display the Add Patient Consent window.

  4. Complete the fields as necessary and click OK.

You can access the Attachments/Consents module from within the Perio Chart module.

  1. Select an entry from the Form drop-down and click the Show History icon () to display the Select Form History window.

  2. Select a listed date and click OK to display the View Form History window.

  3. Review as necessary.
